ALL FOOD VENDORS MUST PROVIDE A COPY OF THE CURRENT YEARS’ BUSINESS OCCUPATIONAL LICENSE, GENERAL LIABILITY INSURANCE*, ADDITIONAL INSURED CERTIFICATES AND CATERER’S LICENSE TO PUFAP BEFORE VENDING IN THE PARK BEGINS.

*If desired, vendors can purchase seasonal insurance through PUFAP for $75/season.

JAZZ IN THE PARK VENDOR GUIDELINES
Please submit your information below. Please read the vendor guidelines above and email, mail, or hand deliver a signed copy.
By submitting this application, you hereby agree to the above conditions and certify that you have thoroughly read the above Jazz In The Park vendor guidelines and understand that they may not be contested once you have signed on to become a vendor at Jazz In The Park. You also understand that People United for Armstrong Park reserves the right to alter these vendor guidelines should they see fit, with a minimum two [2] weeks notice to all participating vendors. You understand that violation of any of the above vendor guidelines may result in my company’s immediate dismissal from Jazz In The Park. with or without opportunity to return.
